How do I make an appointment?  Please call the main number at (708) 603-5980 and have your insurance information available. We will always do our best to accommodate your schedule and urgent care needs.  If you are an established patient online appointments can be requested through the patient portal.

How do I have my medical records transferred?  Click here to print out our Release of Medical Information form.  Fill it out and present it to the health care institution or provider you want to transfer records to/from. It’s often most effective to pick up a copy in person and bring them to your office visit — that way you have your own copy.

Can I see the doctor the same day if I have a problem?  On most days, yes.  We allow for walk-in time slots every day we are in the office.  Please call ahead.

Does the doctor take my insurance?  We take most insurances.  We do not accept most HMO’s, most Medicare HMO’s, or most Medicare Advantage plans.  Please call or go online to your insurance company’s website and see if we are listed as a provider. 

Does the doctor see children?  We see age 16 and above.  Patients under the age of 18 must be accompanied by a parent.

What about prescription refills?  Please ask your pharmacy to submit refill requests electronically (e-scribe) or by fax at 708-589-9059.  Refills requested after hours are done only if emergently needed and at the discretion of the doctor.

How do I reach the doctor after hours?  A physician is on call 24/7.  Call the office number at 708-603-5980.  The call will transfer to and Dr. Rizzo (or the covering physician).
